Claims may only be submitted by sending the Receiver a fully executed Claim Notification Form along with any and all documentation supporting the claim (copy of your CD, emails, cancelled checks, etc.). An email, letter, or phone message conveying this information without a completed Claim Notification Form or supporting documentation (if any) will not be processed as a Claim.
Update (May 1, 2009) – Now available for all persons filing Claims against the Estate is an executable Declaration for submitting to the jurisdiction of the United States District Court for the Northern District of Texas, which is discussed in the Claim Notification Procedures. The Declaration is available by following the link below. Please submit an executed Declaration with your Claim Notification Form, or, if you’ve already submitted your Claim Notification Form, please also submit an executed Declaration via email or United States mail.
